Information Relief Care Bed Program

Description of program
The relief care program provides respite for a period determined in a family unit or other designated caregiver of a dependent person at home. Under this program, a temporary room is provided to the dependent person for a specified period of time for up to thirty days, after which the dependent person will return to his home.
Program Objectives:
• Provide respite to family or persons who provide home care for a person needing assistance with personal care and activities of daily living or day long surveillance.
• Provide a temporary room in a nursing home for people with a relatively stable chronic illness or functional disability who, for a fixed period of between seven and thirty days, after which these people return to their homes.
• Delay to prevent institutionalization of people with chronic illness.
Target population:
• The program is for people with a relatively stable chronic illness or functional disability who, without care needs or specific diagnostic and therapeutic services, need support and monitoring to day activities of daily life and personal care.
• Nursing home relief care beds are reserved mainly, but not exclusively, to people 65 years and older with chronic physical or mental disability or long-term.
• It can or not be of people receiving long-term care services.
Overview:
• The person occupying the relief care bed has most of the services as a regular resident of the Villa receives.
• The maximum occupancy period is thirty days but can be reduced after a stay if a special occasion occurs.
• The cost for using the relief care bed is based on a daily rate determined by the Department and is reviewed every three months. The total payment must be made upon admission.
• The person must send prescriptions renewed for one month of the person admitted to the home before admission. The nursing home will order medications thereafter.
• The person responsible must notify the family doctor of the person admitted that they will continue to be with their doctor, even if the person is occupying the relief bed. Nurses will contact the family doctor as needed during the stay of the person in the home. If the family doctor cannot be contacted, hospital care physician will be contacted.
• The relief care bed is located in a single room so the resident can have more privacy.
• The responsible person shall provide special equipment if necessary for the occupant of the relief bed upon admission. A verification of the equipment in question will be made by the maintenance of the home prior to use.
Steps to follow before admission to conform to the relief care bed program:
• Ask the family physician of the person to make her a physical examination and complete the "examination and medical history" of the Department. The form must be submitted at home seven days before the scheduled admission.
• Complete and return the form "Application" relief program before admission to the front desk (secretary).
• When the nursing home has received the reports, a representative of the nursing home will meet to discuss the modalities of admission and characteristics.
• Read carefully the document "Admission contract relief care program." The contract will be signed during the admission of the person.
• If the necessary documents are not received on time, admission will be delayed and could be subsequently canceled.
• The management of the Villa is available to answer questions of the responsible person or the family before and after admission.
• The responsible person must accompany the person admitted.
• Admissions normally take place at 9:30 am and the person responsible must expect to be present for at least two hours.
